Course, instructor, and TA evaluation forms are now completed online, through Axess. Despite incentives set up by the university to encourage the completion of these forms, it is useful to let your students know the purpose of the forms and to encourage the students to fill them out.

Copies of the original evaluation forms plus a statistical summary of the replies will be available for you on Axess by the beginning of the next quarter. In addition to downloading the PDF summaries for your own Teaching Portfolio, you might like to review the results with the professor you TAed for. Also, the department requires the professor to evaluate the TAs performance at the end of the quarter and provides a special form for this purpose.
